Forms by PACKET
Court filings require statewide and/or court specific forms. Statewide forms are “Judicial Council Forms”.  Court specific forms are “Local Forms”. Some packets may contain both. Civil Filing fees are set according to the Statewide Civil Fee Schedule and the California Rules of Court or state law.
Prior to filing any documents with the court, please check the fee schedule.
 
If you cannot afford the filing fee, you may ask the court to waive the fee by filling out a Fee Waiver Packet. Before deciding to pursue legal action, you should try to work things out with the other party first. If you cannot work things out, seek legal advice from an attorney prior to filing a lawsuit.
 
The following forms packets are provided as a courtesy to you. Forms listed below are the most commonly used forms in each of the specific areas. Please note; there may be other forms you need and some forms may not pertain to your particular case. If you are completing the forms yourself, you should have an attorney or the self help center review them PRIOR to filing them with the clerk.
